Antrim Area Hospital discharged a patient from its Emergency Department despite his poor health. His daughter believed he should have been admitted to the hospital for further treatment.

The Western Trust’s failure to give a patient an MRI test caused her to experience prolonged pain and unnecessarily delayed treatment of her endometriosis.

A woman complained that her IVF treatment failed because the Belfast Trust’s fertility clinic failed to monitor her progesterone levels. Our investigation did not identify any failings.

A man said a hospital should not have discharged him without doing further tests on a cyst on his brain. We found he was treated appropriately.

The Northern Trust should have told a patient and his family that he would not be resuscitated if his heart or breathing stopped. 

A woman complained that doctors did not do enough to monitor and investigate her father’s condition prior to his death.

A woman believed that the Western Trust should not have left it to her family to arrange palliative care for her elderly mother.

A patient complained he was uncomfortable after he was fitted with a catheter following surgery.  We found no failings in his care and treatment.
