The Board of Governors of a secondary school should have set up a sub-committee to investigate a complaint about whether a pupil’s A Level grades were estimated fairly.
A woman complained that her father’s health deteriorated because he was not looked after properly in his care home. Our investigation found failings in his care and treatment, as well as a string of serious record keeping failures by the home.
The Ombudsman has found that an RQIA inspection and subsequent report was completed according to relevant standards, but that it should have taken further steps to provide the registered manager of a care home with a copy of the draft report so that she could comment on its factual accuracy.
