Failings over Serious Adverse Incident investigation
The Western Trust didn’t communicate properly with a woman during its investigation into the death of her husband.

We asked the Trust to reach agreement with the complainant over which parts of the draft Serious Adverse Incident (SAI) investigation report she wished to provide responses to. Following this it should ensure it has fully and clearly responded to each of her concerns.
We also asked it to review its procedures for updating service users during the SAI investigations process.
Given that we were concerned about how the Trust engaged with our Office during our investigation, we also reminded it of it's responsibility to respond appropriately to our enquiries in the future.
