A woman complained that her father’s health deteriorated because he was not looked after properly in his care home. Our investigation found failings in his care and treatment, as well as a string of serious record keeping failures by the home.
We found that a medical assessment performed during a home visit was carried out in line with good medical practice, but that the GP should have recorded what had been discussed with the patient and her family, and what they should do if her symptoms did not improve.
An investigation has revealed that a nursing home failed to fully record its observations of a resident's head injury following a fall, and did not call an ambulance to take him to hospital until almost eight hours after the incident.
