The Chief Executives of three health trusts have apologised after an error in recording a patient’s date of birth led to him being confused with another patient.

The Northern Ireland Ambulance Service Trust is considering suitable equipment for transporting new-born infants in ambulances after an Ombudsman's report recommended it apologise to a complainant.

The investigation looked at the actions of the Northern Ireland Ambulance Service when responding to a 999 call after the complainant’s father had suffered a stroke.

An investigation has concluded that the care plans prepared for a resident of a nursing home in Armagh were largely adequate, but that it had underestimated the resident’s risk of a fall, and that it was inconsistent in its assessment of his mental state.
