Communication failures meant patient had to wait for treatment
A man said he didn't realise the South Eastern Trust had discharged him from its mental health services. The resulting confusion without left him without treatment for 18 months.

In its response to our draft report the Trust outlined several steps it could take to improve its service and prevent the failures identified in this report from happening again. This included improving its digital records system, seeking regular updates from community and voluntary organisations about waiting times, and creating a leaflet to explain the role of the MHAC to patients. We welcomed this learning.
