Trust followed relevant guidelines in interpretation of MRI scan

A man complained that an MRI scan carried out by the Belfast Trust failed to identify the cause of his back pain. 

We did not uphold the complaint.

GP practice should have referred patient for further tests

A woman claimed that if her late husband’s weight loss been investigated properly his cancer may have been detected sooner. 

We found it was a ‘significant failure in care and treatment’ that his GP didn’t send him for further diagnostic treatment.

Commissioner's Guidance - June 2025

The Commissioner has today published a revised version of The Guidance on the Code of Conduct for Councillors, a publication for councillors to help them better understand their obligations under the Code.

The Guidance, which was first produced in 2017, was developed with input from councillors and others to make it useful and relevant.

The latest version comes in advance of a more detailed review and re-launch of the publication later this year.

Councillor Wesley Irvine (Ards & North Down Borough Council) has been suspended for three months following an Adjudication Hearing held on 13 June.

Ombudsman begins investigation into Northern Ireland Housing Executive

The Ombudsman has written to the Chief Executive of the Northern Ireland Housing Executive to say she has commenced an 'own initiative' investigation into the organisation.

The decision follows the Ombudsman’s consideration of the Housing Executive’s response to her investigation proposal from earlier this year.
