Investigation into complaint over cause of patient’s sudden death

We looked at whether an alleged failure by the Trust to treat a cyst on a patient's chest led to him developing sepsis.

Although we found some failings, we concluded there was no negative impact on his overall care, which was reasonable and in line with relevant standards.

Report recommendations

We asked the Trust to apologise to the complainant for the failure of nursing staff to bring the cyst to the attention of medical staff.
