Failings led to seven month delay in patient receiving cancer diagnosis

The Southern Trust failed to appropriately triage a referral from a patient's GP.  It also did not tell the patient or her GP of its decision to downgrade the referral from urgent to routine.

Report recommendations

We asked the Trust to apologise to the patient.

We also asked it to train staff on the importance of considering all information presented on a referral form when making triage decisions. This should make sure staff are aware of the need to make a note of their rationale for these decisions and retain it within the patient’s records.
